What you should take with you to the hospital

  • Identity card.
  • Insurance certificate from the relevant health insurance company.
  • Medical documentation and recommendation for hospitalisation (this does not apply in case of an acute disease).
  • X-ray images from the recent examinations before hospitalisation.
  • Working incapability document if any has been issued by the attending physician.
  • If you are registered at the Job Office, the relevant document issued by the office.
  • Personal hygiene items (soap, toothbrush, toothpaste, shampoo, toilet paper, house shoes, bathrobe, you can have your own pyjamas as well).

In addition, the following items should be taken into the maternity hospital

  • Pregnancy certificate (and all substantial ultrasound and other examinations).
  • Recommendations, examinations of medical specialists.
  • Married women - certificate of marriage.
  • Single women - agreement on surname of the child (where the woman wishes that the child and the father have identical surname). If such agreement is not submitted, the child's surnames remain the same as the mother's.
  • Divorced women - award of the court with legal force clause of the award (if the child is born within 300 days from the divorce, the marriage certificate or paternity determination record is necessary as well).
  • Widowed women - husband's certificate of death (if the child is born within 300 days from the husband's death, the marriage certificate or paternity determination record is necessary as well).
  • BEPANTHEN - cream.
  • Drink - water (no bubbles), tea.
  • Washable shoes, socks, if possible also night gown and bathrobe (hospital clothes are available at the department).
  • Maternity bras, bra inserts, elastic mesh pants.
  • Paper nappies, cleaning tissues.
  • Children's layette for the way home.
  • YAL - gel (can be used instead of clysma, available on prescription only).
  • Relaxation music - cassette (the player is available in the room, CD.
The father - light and comfortable clothes, house shoes.
